Why we’re building Praxis

The patient carries the continuity.

After 20 years on active duty and now receiving care through the VA, I see a persistent problem: the patient carries the continuity, even when the healthcare system doesn’t. Assignments change, providers rotate and care moves between facilities. Patients often have to rebuild their story at the next appointment.

Medical records don’t capture everything between visits - when a symptom started, how it changed or what happened after a medication change. Reconstructing those details takes time from patients and care teams, especially when nurses are stretched thin.

We’re building Praxis to bring that context forward and extend nursing expertise through robotics. Our goal is to help teams see more patients and move appropriate evaluation and treatment forward sooner, including within the same visit when clinically appropriate and resources allow.

Three parts, one connected solution.

TALON - Taskflow for Agentic Logic, Orchestration and Networking - is the technology platform powering the patient-facing TALON patient app, the Praxis nurse workspace / VR interface and the Praxis robot. We’re developing them together so patient context follows the encounter.

Patients capture symptoms, medication updates and changes between visits, and authorize sharing relevant information for an encounter.

Praxis nurse workspace / VR interface

Nurses review intake and authorized medical history in a source-linked draft, alongside an immersive interface for remote participation and permitted robot control.

Praxis robot

A mobile physical interface connects patients and remote nurses, combining bounded autonomous workflow support with authorized VR teleoperation.

Bounded autonomous mode

Designed to perform approved tasks within configured operating limits, escalating situations that require human involvement.

Authorized VR teleoperation mode

An authorized nurse would view the robot’s surroundings and control permitted movements through an immersive interface, alongside intake information and relevant medical history.

Explicit handoff determines which mode controls the robot. Local motion safeguards remain active in both modes. Clinical assessment and decisions remain with licensed professionals.

Where we start

Our intended first use case is nurse-led intake and remote assistance for hospitals and outpatient care teams. A mobile physical interface would let remote staff reposition it during an encounter, while bounded autonomous functions support routine workflow. The value beyond fixed video or tablets remains something we will evaluate.

We’re building toward this model. Assignments would follow applicable nursing licenses and facility credentialing. Onsite teams would continue to provide hands-on care and immediate assistance.
